DO YOU WANT TO REFER A CLIENT?

In the first instance any client or professional looking to refer to any of our services will be required to complete a short application form which can be downloaded in either Word or PDF Format by clicking on the links below:

Alternately you can ask for an application to be emailed or posted to you by calling our admissions team on (01752) 797102 or emailing: admissions@broadreach-house.org.uk

We can also accept a CCA in place of an application form where one is available.

On recipient of a completed application form a member of our centralised admissions team will make contact to arrange a convenient time and date for assessment which can be conducted face to face, by telephone or skype.

Where the needs of the client are more complex such as co-morbidity, dual diagnosis, pregnancy, and criminal justice involvement, it is helpful to speed this process if a detailed history is provided alongside the most recent psychiatric/medical reports when the referral is made. This minimises the risk of unnecessary delays in decision making and the admission process, which has a primary focus on risk assessment and risk management planning.

On the very rare occasion we feel we are not able to help we will provide a written explanation and recommendations.
